Letter from 2nd parent for minor child H4 visa

Hi All,

Me and my wife both are on H1 in USA. My wife is going to India to bring our daughter (2.5 years old) after getting H4 visa to her.

The following question is mentioned in FAQ section of Chennai consulate.

How do I get a visa for my minor child?
The minor children must be accompanied to the interview by the parents. If only one parent can attend the interview, the other parent must send a copy of the absent parent's passport along with a signed, notarized, letter of consent stating that the minor children have permission to apply for a visa for travel to the USA.

Where can I get format of this letter of consent? How can I notarize it?

Please let me know if any boday had similar situation.

Write a simple letter sating that the child is appearing and applying for the visa with your permission and knoweldge and you have no objection.
you can get the document notarised in USA itself , by any Notary.( Search for Notray service in google for nearby area you live)
