Have you ever applied for employment authorization from USCIS?

In the follow up question it ask for Date(s).

Is it the "Received Date" or "Notice Date" on I-797 or Is it the "Initation Date" related to approved period on EAD card?

I did a paper filing, used "Received Date" in Q11 of I 765 form. I got my EAD on Jul 11.

which service center you filed,total how many days it took for approvel and is it 1 year EAD or 2 years you got?.Throw some light dude.

I applied in NSC. Sent my application via paper filing on Jun 9 and got 2 yrs EAD on July 11.
