In the recent past, there have been some changes to the DS-160 Visa Application form where applicants applying for US Visa are no longer required to upload a digital photograph while filling up the DS160 form. These applicants must set up an additional biometric appointment at the Visa application center (VAS) or Offsite Facilitation Center (OFC) before the US Visa Interview at the US Consulate or Embassy.

Applications from certain countries do not require to upload a digital photograph while filling out the DS-160 Visa Application Form. US Embassy/Consulates may require these visa applicants to take a Biometric Appointment for Fingerprinting & Digital Photograph prior to the visa interview or may ask applicants to bring a 2 inch by 2 inch photograph at the time of interview.
During the Biometric interview process, biometric information including fingerprints and digital photographs will be collected at the VAC or OFC. Applicants for U.S. visas are required to appear in person for an appointment at the VAC and the visa interview at the U.S. Embassy or Consulate.

This process is not application to applicants from all countries. Applicants only from certain countries are required to follow this process. Below is the list of US Embassies and Consulates in the world that may require you to go to a VAC center for biometric or the Embassy or Consulate may ask you to bring 2 inch x 2 inch passport size photograph with a white background. You may no longer be asked to upload a digital photograph while filling out a DS-160 form when applying at these embassies or consulates.
