Persons applying for renunciation of Indian citizenship must submit the required photographs, documents, and fees with their application. The required documents for renunciation of Indian citizenship depend on the applicant’s renunciation category.

After gathering and compiling all of their required documents and fees, applicants should identify their consular jurisdiction and service center to which they should send their application.

Renunciation Naturalized before June 1, 2010 with Indian Passport

If your renunciation was naturalized before June 1, 2010, you must submit the following documents with your renunciation of Indian citizenship application.

  • 2 completed Renunciation Certificate Forms.
  • Signed CKGS Disclaimer Form.
  • Copy of Naturalization Certificate.
  • Completed Additional Particulars Form (only for San Francisco, New York, and Chicago jurisdiction).
  • Most recent Indian Passport.
  • Photocopies of first 3 and last 2 pages of your most recent Indian passport.
  • Duly filled and signed Non US Citizen Form (only for non US passport holders applying from Houston jurisdiction).
  • Document showing legal status of applicant such as copy of permanent resident card (both sides) or a copy of page showing valid visa status (only for non US passport holders).
  • Copy of information page and last two amendment pages of applicant’s U.S. or foreign passport.
  • Name change document such as a copy of marriage certificate or legal court order if applicant’s name in Indian government issued document is different from U.S. or foreign government issued document.
  • Proof of address such as a copy of state issued ID, driver’s license, utility bills, or valid rental lease signed by applicant and landlord.
  • 2 copies of signed checklist (for walk-in applicants) or 1 copy of signed checklist (for postal applicants) with check-marked boxes and proof of payment of CKGS courier service charges.
  • For return of documents, prepaid envelope with Airway Bill Number with service provider name.

Renunciation Naturalized on or after June 1, 2010 with Indian Passport

If your renunciation was naturalized on or after June 1, 2010, you must submit the following documents with your renunciation of Indian citizenship application.

  • 2 completed Renunciation Certificate Forms.
  • Signed CKGS Disclaimer Form.
  • Copy of Naturalization Certificate.
  • Completed Additional Particulars Form (only for San Francisco, New York, and Chicago jurisdiction).
  • Most recent Indian Passport.
  • Photocopies of first 3 and last 2 pages of most recent Indian passport.
  • Duly filled and signed Non US Citizen Form (only for non US passport holders applying from Houston jurisdiction).
  • Document showing legal status of applicant such as copy of permanent resident card (both sides) or a copy of page showing valid visa status (only for non US passport holders).

Applicants of either of the above categories who were naturalized without an Indian passport must submit the following additional documents.

  • 2 completed Renunciation Declaration Forms
  • Notarized Affidavit of Loss signed by the applicant
  • Police report for the lost Indian passport (for applicants applying from Houston jurisdiction only)
  • Proof of Indian origin such as copy of Indian election card/Aadhar card/Ration card or any two of the following documents:
    • PAN card
    • Driving License
    • Indian birth certificate
    • Domicile certificate
    • School Leaving certificate
    • Other document showing proof of former Indian citizenship

Minor applicants of either of the above categories must submit the following additional documents.

  • 2 completed Renunciation Certificate Forms (if possessing Indian passport) or Renunciation Declaration Forms (without Indian passport) signed by the minor child over 5 years old and by both the parents. Signature of both parents must match their passport signatures. If minor is under 5 years old, thumb impression of minor child and signatures of both the parents are required.
  • If minor applicant does not possess a naturalization certificate and gained naturalization through his/her parents, copies of naturalization certificates of both parents are required.
  • A notarized statement signed by both parents explaining why the child does not possess a separate naturalization certificate.
  • Additional Particulars Form signed by both parents on behalf of minor (only for San Francisco, New York, and Chicago jurisdiction).
  • Non US citizen form must have signature of minor child if over 5 years and thumb impression if under 5 years along with signatures of both parents (only for non US passport holders applying from Houston jurisdiction).
  • 2 copies of signed checklist (for walk-in applicants) or 1 copy of signed checklist (for postal applicants) with check-marked boxes and proof of payment of CKGS courier service charges.

Renunciation Naturalized 10 Years Back without Indian Passport

If your renunciation was naturalized 10 years back and you are not in possession of your Indian passport, you must submit the following documents with your renunciation of Indian citizenship application.

  • 2 completed Deemed Surrender Certificate Forms.
  • Signed CKGS Disclaimer Form.
  • Copy of Naturalization Certificate.
  • Completed Additional Particulars Form (only for San Francisco, New York, and Chicago jurisdiction).
  • Notarized Affidavit of Loss Form.
  • Duly filled and signed Non US citizen form (only for Non US passport holders applying from Houston jurisdiction).
  • Document showing legal status of applicant such as copy of permanent resident card (both sides) or a copy of page showing valid visa status (only for non US passport holders)
  • Copy of information page and last two amendment pages of applicant’s U.S. or foreign passport.

  • 2 completed Deemed Surrender Certificate Forms signed by the minor child over 5 years old and both parents. Signature of both parents must match their passport signatures. If minor is under 5 years old, thumb impression of minor child and signatures of both the parents are required.
  • If minor applicant does not possess a naturalization certificate and gained naturalization through his/her parents, copies of naturalization certificates of both parents are required.

A reference fee of $20 may apply to applicants who are Non US passport holders and Non US citizens residing in US for less than 2 years.
